چکیده
A 73 year old man was admitted with progressive dyspnoea. His past medical history included peripheral artery disease (intermittent claudication of lower extremities) and ischaemic heart disease (angina pectoris). Two months before renovascular hypertension was diagnosed after he presented severe hypertension and mild renal insuYciency (plasma creatinine concentration 150.3 μmol/l). Renal angiography showed a solitary stenosis of his left renal artery. He had undergone a renal angioplasty two weeks before admission. On examination he had signs consistent with cardiac failure with pulmonary oedema; systolic blood pressure was 180 mm Hg and diastolic 100 mm Hg. A reticulated reddish mottling of the skin was seen on his feet (fig 1). His plasma creatinine concentration was 353.6 μmol/l. A skin biopsy specimen revealed needle-shaped crystals in his small arteries (fig 2). The patient’s condition continued to deteriorate; he was treated with vasodilators, diuretics, and pentoxifilline but developed progressive renal insuYciency and refractory heart failure and died 10 days after admission.

Livedo Reticularis: An Enigma.
Livedo reticularis is a common cutaneous manifestation of APS and may be a prognostic marker of more severe disease. It is associated with arterial and venous thrombosis and pregnancy morbidity irrespective of the presence of antiphospholipid antibodies. Recent results suggest the possibility of an association with accelerated atherosclerosis in patients with livedo. Given the similarities betw...
متن کاملLivedo Reticularis: A Rare Skin Manifestation of Pancreatitis
Skin manifestation associated with pancreatitis can have various presentations. Skin manifestations such as Grey Turner sign or Cullen's sign are known and well documented findings of pancreatitis and carry significant prognostic value. However, livedo reticularis is a rare phenomenon in setting of pancreatitis. Here we report a case of 40 year old female who experiences recurrent episodes of l...
متن کاملAmantadine-induced livedo reticularis - Case report*
Livedo reticularis is a spastic-anatomical condition of the small vessels which translates morphologically by a reticular pattern, interspersing cyanosis, pallor and erythema. The same can be congenital or acquired. Among the acquired, we highlight the physiological livedo reticularis and the idiopathic livedo by vasospasm; the latter configures the most common cause. The drug-induced type is l...
متن کاملDifferences in anti-phosphatidylserine-prothrombin complex antibodies and cutaneous vasculitis between regular livedo reticularis and livedo racemosa.
OBJECTIVES We examined the prevalence of LAC, aCL antibodies (Abs), anti-beta(2)-glycoprotein I (anti-beta(2)GPI) Abs and anti-phosphatidylserine-prothrombin complex (anti-PS/PT) Abs in patients with regular livedo reticularis or with livedo racemosa to determine whether those Abs correlate with the clinical or serological features. Assuming that a correlation exists, early recognition of the s...
متن کاملThe Role of Statins on the Outcomes on the Cholesterol Embolism Syndrome with Secondary Vasculitis - A Case Report and Review of the Literature
The cholesterol embolization syndrome (CES) is a potentially severe complication of atherosclerosis, caused by embolization of atherosclerotic plaques, most frequently after vascular surgery or endovascular procedures. The clinical presentation closely mimics that of systemic vasculitides hence it should be considered in the differential diagnosis in patients with suspected vasculitis. Skin les...
